Chronic Disease Management Care Plan

Patients with Diabetes are entitled to five visits with an Allied Health Professional

under Medicare's Chronic Disease Management framework.

 

Prior to your appointment please see your GP who can provide you

with a care plan with visits to see a Diabetes Educator.

Private Health Insurance

  • Please check prior to your appointment if your Private Health fund provides any rebate for Diabetes Education
