Initial Consultation (60 mins)
Take the first step toward confident diabetes management. In this comprehensive session, we will review your medical history, discuss your current diabetes management and lifestyle, and explore the latest diabetes technologies (such as continuous glucose monitors, Automated insulin delivery systems and insulin pumps) to see what fits your lifestyle. Together, we will create a personalised management plan tailored to your goals.
Cost: $185.
Medicare Rebates: To claim a Medicare rebate for this service, you must have a valid Chronic Disease Management Plan (CDMP) from your GP. Please note that if you do not have this care plan in place before your session, you cannot access the rebate. Private health insurance extras may also provide a rebate - please check with your insurer prior to confirm.
Please bring: Your GP referral/care plan (if applicable), recent blood test results, current medications, and upload any continuous glucose monitoring devices or insulin pumps you currently use.
Transition safely and confidently to your new insulin pump with a fully supported clinical package. This service encompasses your comprehensive (up to) 2-hour practical setup session (configuring settings, device insertion, and dose navigation), as well as all essential pre- and post-appointment care.
What’s Included: Pre-appointment liaison with your Endocrinologist to finalise your starting doses, management of your pump ordering paperwork, the 2-hour intensive training session, and regular clinical check-ins during your first 1–2 weeks on the new device.
Cost: $550 (inclusive of all pre-appointment coordination, the 2-hour session, and 1–2 weeks of initial follow-up contact).
Medicare Rebates: A valid Chronic Disease Management Plan (CDMP) referral from your GP is strictly required to claim a Medicare rebate. Without this pre-arranged care plan in place, you cannot access the rebate. Private health insurance rebates may apply depending on your policy.
Please bring: Your fast-acting insulin, any pre pump instructions emailed, your current glucose monitor, and your GP care plan referral.
Review & Progress Consultations
Regular reviews are essential to safely manage your diabetes, fine-tune your plan, and keep your technology optimized. As a general guide, routine reviews are recommended every 3 to 6 months, or sooner if you have recently started an insulin pump or new medication.
Please choose from the two booking options below based on your current needs:
Standard Review (30 mins) | $120
Best for routine 3-to-6-month check-ins, straightforward data downloads, or quick adjustments to your current management plan.Extended Review (45 mins) | $150
Recommended if it has been over 6 months since your last visit, if you are experiencing frequent hypos/hypers, or if you need extra time to troubleshoot your pump or CGM technology.Medicare Rebates: A valid GP Chronic Disease Management Plan (CDMP) is strictly required to claim a Medicare rebate. Without this active plan in place before your appointment, you cannot access the rebate.
